Select one membership plan at enrollment:
- Essential Membership — $199 per month. Three-month initial commitment. Includes one Essential IV each month, one standard IV add-on, 10% off eligible additional IV therapy and wellness injections, and 10% off eligible peptide management fees. One unused monthly IV may roll over for one month.
- Signature Plus Membership — $229 per month. Six-month initial commitment. Includes one eligible standard IV each month, one complimentary wellness injection, 15% off eligible additional IVs, injections, and add-ons, and 10% off eligible peptide, hormone, and weight-management service fees. Athlete is available with a $20 upgrade. One unused monthly IV may roll over for one month.
- Premium Membership — $299 per month. Twelve-month initial commitment. Each month, choose one eligible standard IV, including Athlete, or one 250 mg NAD+ infusion. Also includes one complimentary wellness injection, two standard IV add-ons, 20% off eligible additional IVs and NAD+ treatments, 15% off eligible wellness injections, and 10% off eligible peptide, hormone, and weight-management service fees. Unused monthly benefits may roll over for up to two months, subject to the signed agreement.
Membership term and billing. Membership fees are billed monthly through Square using the payment method provided at enrollment. The selected plan remains active for its stated initial commitment period. The final Square subscription schedule and signed membership agreement govern billing dates and payment processing.
Monthly benefits. Benefits are personal, nontransferable, have no cash value, and are subject to appointment availability, clinical eligibility, product availability, and rollover limits stated in the signed agreement.
Clinical eligibility. Every IV, NAD+, peptide, injection, prescription, hormone, and weight-management service requires appropriate screening, clinical assessment, and provider approval.
Discounts and exclusions. Discounts apply only to eligible services and cannot be combined with other promotions unless approved in writing. Prescription medications, pharmacy costs, laboratory testing, shipping, consultations, mobile-service charges, after-hours premiums, holiday fees, cancellation fees, missed-appointment fees, and third-party charges may be excluded unless specifically included.
Authorization and acknowledgment. By enrolling, the member acknowledges the selected monthly fee, commitment period, included benefit, applicable discounts, recurring billing authorization, rollover rules, and all terms contained in the signed membership agreement and Square checkout disclosures.
Final controlling documents. This website is a summary only. The signed membership agreement, treatment-specific consent forms, clinical documentation, and Square subscription terms control if any wording differs.
